Who is eligible for case management?

To be eligible for Case Management at APO a person must be living with HIV, live in one or our 29 counties, and meet income guidelines.

What is the Purpose of Case Management?

Case management aims to identify and address barriers in a client’s life and to ensure access to HIV medications and medical care.

Case management offers a comprehensive range of services, including financial assistance for medications as well as assistance with medical, psychiatric, mental health, and dental care. Housing and utility assistance are available but limited.  Additional services include, adherence counseling and education, referrals to community resources, and transportation services.    Peer support services are available through support groups and a variety of events throughout the year.
