Linkage to Care (LTC) is an intensive short term case management program designed to assist individuals newly diagnosed with HIV with establishing medical care. LTC Case Managers work with clients to identify, address, and reduce or eliminate barriers to accessing medical care and medications while also providing education and support to both the newly diagnosed individual and their family. The goal of the Linkage to Care Program is to connect individuals with HIV medical care and treatment as soon as possible after receiving an HIV diagnosis.
